Evaluations will be completed in the order they are scheduled. Should their be a waitlist at the time of contact, you will be added to the list during your initial call and someone will be in touch with you when an opening becomes available.
Paperwork will be sent to you via email after an appointment has been created. In this email you will receive a link to the patient portal and guided through all necessary forms. Should we need additional information from you prior to sending paperwork, you will be contacted via email It is your responsibility to respond with information needed in order to move forward with your evaluation.
All completed paperwork must be received by our office 48 hours before your scheduled appointment. If paperwork is not received, we reserve the right to cancel your evaluation and reschedule once paperwork has been received.
Please arrive on time for your scheduled appointment and bring any necessary items required for the evaluation.
After your evaluation, your therapist will highlight their findings with you and determine if therapy is needed. You will also receive a written report from your therapist via our patient portal for your records.
