New Patients:
Evaluations will be completed in the order they are scheduled. Should their be a waitlist at the time of contact, you will be added to the list during your initial call and someone will be in touch with you when an opening becomes available.
An appointment time for your evaluation will be marked for you and paperwork will be sent to you via email. In this email you will receive a link to the patient portal and guided through all necessary forms. It is your responsibility to check your emails promptly and complete all forms. Your appointment is not confirmed until all paperwork is received in our office. Without completed paperwork, we reserve the right to cancel your evaluation and reschedule once paperwork has been received.

All existing patients will have access to our patient portal, Fusion Web Clinic. This portal will allow you to access your child's reports, progress summaries throughout treatment, and invoices/receipts from payments for services.
**All patient primary contacts will provide their email address and phone number within the patient portal. This information will be used as a way to directly contact you regarding your child's therapy appointments, receive information regarding office closures/delays, receive updated privacy policy notices, and view documents related to your child's progress.
