Our practice utilizes an Electronic Medical Record (EMR) system called FUSION WEB CLINIC. If you are a new patient, please note that all of our patient forms will be submitted to you via our Patient Portal upon scheduling your initial appointment. Please confirm with the office if you are unable to find these forms in your email. Please check your junk/spam mail and ensure that all future emails from FUSION are routed to your inbox.
After clicking the link in the initial email, you will be prompted to create a username (e.g. your email address) and a secure password to access the portal. You will need to remember this information, as this is how you will access the patient portal moving forward. The link to create this does expire, so please do this upon receipt of the initial email.
